Proof of COVID-19 Vaccinations

The following participants and members are required to show proof of full vaccination by October 31, 2021, with their first dose received by September 17, 2021: - Players born in 2009 or earlier (this includes players born in 2009 that have not yet turned 12) - Game officials (referees, linespersons, and timekeepers) - Staff - All team officials (coaches, trainers, managers) - Volunteers

If you have not received your second dose, please submit the receipt for your first dose and provide the date scheduled for your second dose below. Once complete, the new receipt can be resubmitted with both doses.
