The 'Top Shelf' Bench Staff of the Year Nomination Form

This award recognizes the exceptional contributions of an individual member of a Bench Staff (coach, assistant coach, staff, trainer, manager), OR an entire Bench Staff, in the Collingwood Girls Hockey Association who has gone above and beyond to make a lasting impact through their commitment and dedication to their respective role(s) on the team.

The recipient(s) has made a memorable impact on the overall hockey experience for their team through their demonstration of respect for players, officials, opponents and parents; their leadership and positive influence on players’ growth as hockey players and people; and their balanced approach between skill building, competitive play, effective communication, and fostering a love for the game of hockey.

Any and all persons associated with the Collingwood Girls Hockey Association is eligible to submit a nomination for The 'Top Shelf' Bench Staff of the Year Award.
