Out of Province COVID-19 Vaccine Registration 

What is registration for?

Some individuals may have received partial or complete vaccination out of province. By filling out this form you are starting the registration process for any doses of a COVID-19 vaccine received outside of Ontario, and are consenting to follow up by the Peterborough Public Health vaccine team. COVID-19 vaccinations will be recorded in COVaxON, the provincial record management system.

Valid record of a first dose is preferred before a second dose appointment can be booked in Ontario. Records of vaccination will also help track rates of 1st and 2nd dose coverage across Ontario. Registration on COVaxON will ensure you can easily access valid proof of your COVID-19 immunizations when needed (e.g. for/when travelling).

Individuals that have not received vaccination with a Health Canada authorized vaccine may also be eligible for an additional dose of an mRNA vaccine. This is important given the proof of vaccine requirements currently in effect in Ontario. Please note that doses received while serving in the Canadian Armed Forces DO NOT need to registered in COVaxON. The QR code generated through the federal government is sufficient proof of vaccination.

How do I register?

If this is a service you require and you are a resident of the Peterborough City-County Health Unit region, please complete all required fields on the following forms. Please be sure to include phone number and email address. This will ensure that we will be able to contact you once registration is completed, or if additional information is necessary. You will be asked to provide some basic demographic information, along with proof of address (such as a driver's license or mail addressed to you), proof of vaccination (such as a vaccine receipt, card, or certificate), and proof of travel (such as hotel confirmations or flight itineraries) that will be uploaded. There will be several pages to complete. Please make sure you fill out all required fields and click next at the bottom of the page.

If you do not have proof of vaccination available, please contact the location that administered your COVID-19 vaccine to receive a copy of the details before completing this form. Please ensure that your proof of vaccination is an official document from the issuing country/province/region, which includes the following:

• First and Last Name
• Vaccine Type
• Vaccine Lot Number
• Date of Vaccination
• QR Code (if issued from Alberta)

**Please note that an in-depth review of all submitted documents will take place. It may take up to 30 days to receive a request for additional information, or for registration to be completed. Please only complete this survey once.
