Transitional Care with Health Literacy Driving Quality

This presentation will discuss the evidence behind Transitional Care Management and the impact that health literacy can have to drive quality outcomes in everyday inpatient and outpatient settings.  
 

At the time of discharge, conducting a needs assessment identifies risk factors that must be addressed to ensure a safe and effective transition in care to an outpatient setting. This shift is impacted by health literacy. Health literacy is the capacity to understand key health information and make appropriate health decisions. Tens of millions of Americans have limited health literacy—a fact that poses major challenges for the delivery of high-quality care. Low health literacy is associated with poor health outcomes, impacting the ‘Triple Aim’ framework of experience of care, population health and cost of care.

This program has been approved by the National Association for Healthcare Quality for a maximum of 1 CPHQ continuing education credits for this event.

 

 

Dr. Jayasuriya has a decade of experience working as a hospitalist, outpatient clinician, health administrator, and health coach. Dr. Hershey used her experience to start BridgeCare in 2020.  BridgeCare strives to overcome specific challenges within the health care system that affects patients, systems, and payers every day. Knowing many patients go back and forth to hospitals, acquiring large bills, and affecting the quality of their life, BridgeCare aims to fill gaps in care to help prevent patients from these stresses. Through BridgeCare, outpatient providers now have the assistance to improve their quality scores and day-to-day management load, at no cost to them.
