Target Audience

Eligible individuals must have 1) taken the VA, Penn, or Ryerson University Basic CBT-I workshops and, 2) treated 5 to 10 patients. Others may be eligible but this requires a clear demonstration of formal training (e.g., a rotation during internship that focused on the assessment and treatment of insomnia) and once again the treatment of 5 to 10 patients with CBT-I. Those who wish to establish eligibility should write to Michael Perlis at mperlis@upenn.edu.

We would like to ask for attendees to consider submitting a case study report for this course.

Why should you do such a crazy thing? Well, if you elect to submit a case (and do so on or before the deadline), we will rebate you $50 following your attendance at the Advanced Course. But wait, there’s more! If you elect not only to submit, but also to present the case (and field questions and comments at the course) and submit a Power Point presentation outlining the case on or before the deadline, we will rebate you an additional $50 (total $100) following your attendance at the Advanced Course. Please know that these presentations are meant to be instructive for you and the other attendees. There will be no harsh judgments, tomatoes thrown, etc. These presentations form the core of what the Advanced Course is about and we greatly appreciate the effort expended by attendees who join us as presenters. Finally, as a testament to the value of this component of the course, the majority of last year’s attendees agreed that the case presentations were the highlight of the Advanced Course. So please don’t be bashful. This is not required, but we’d like to have up to 10 attendees submit cases.

The case submission should be:
  1. Tendered through the Case Studies Archive worksheet (8 weeks prior to the course) (see www.med.upenn.edu.cbti/studies/html, and
  2. Formatted based on a Power Point template we will provide upon submission/request (email mperlis@upenn.edu). This is due 4 weeks before the course.

It’s that simple! If more than 10 cases are submitted, presentations will be selected based upon the date of submission and how the case fits into the teaching mission. So sign up now!

Objectives:

Upon completion of this course, participants should be able to:

Day 1:
  1. Articulate the advanced models of insomnia as it relates to clinical practice.
  2. Integrate an understanding of the advanced models of insomnia into clinical practice as a means for addressing patient resistance.
  3. Evaluate the relative merits of various modes of CBT-I treatment delivery including in person, group treatment, and tele-health technologies.
  4. Analyze the possible security risks involved in telehealth delivery of CBT-I in order to protect patient privacy.
  5. Describe how actigraphy can be integrated into clinical practice to improve initial assessment treatment and outcomes.
  6. Analyze the relative merits of various actigraphy features and how they can be utilized in practical applications.
  7. Specify when to utilize actigraphy in the practice of CBT-I and why the data serves as an adjunct to sleep diary data.

Day 2:
  1. Explore the use of adjuvant and alternative treatment strategies that can be used to modify or substitute for CBT-I.
  2. Integrate phototherapy with CBT-I in order to address circadian rhythm issues that complicate the treatment of insomnia.
  3. Articulate how mindfulness meditation skills can be integrated into the practice of CBT-I to improve TX outcomes.
  4. Explore how to synthesize data from a PSG report and integrate into clinical treatment choices.
  5. Differentiate between Apnea/Hypopnea Index, Apnea Index, and Periodic Limb Movement Index, and understand how to integrate these measures into the assessment of intrinsic sleep comorbidities.
  6. Integrate validated sleep instruments into the assessment of insomnia to help inform choice of clinical treatment interventions.
  7. Articulate the behavioral treatments for SDB & PLMs and integrate into CBT-I.
  8. Evaluate 3 options for hypnotic medication tapering for the purpose of treatment planning.
  9. Support clients in overcoming barriers that impede willingness to taper from hypnotic medication.
  10. Analyze the relative merits and clinical implications of utilizing hypnotics concurrently with CBT-I.
  11. Analyze the relative merits of utilizing stimulant medication such as modafinil concurrently with CBT-I, and its clinical implications.
Day 3:
  1. Articulate the components of a clinical assessment algorithm and its clinical implications in the treatment of insomnia.
  2. Apply the clinical algorithm for case presentation and clinical decision making for insomnia patients.
  3. Evaluate patients for appropriateness for CBT-I treatment, and specify comorbidities that are contraindicated.
  4. Present clinical case information in order to formulate and evaluate treatment options and outcomes.
  5. Write a data driven clinical insomnia case assessment and outcome report to be shared with other health care professionals.
  6. Evaluate common issues related to patient resistance and non-adherence to CBT-I treatment recommendations.
  7. Utilize cognitive strategies to overcome insomnia patient anxiety and resistance to change.

MICHAEL PERLIS, PHD

Michael L. Perlis, PhD is internationally known for his work in the area of Behavioral Sleep Medicine (BSM). He is a coauthor of the first text book in this field and he is the senior author of a published CBT-I treatment manual and a larger text summarizing all BSM treatments. His research includes neurocognitive phenomena in insomnia, the cognitive and/or behavior effects of sedative hypnotics and placebos, the development of alternative treatment approaches for insomnia, and sleep in depression. His work has been, and continues to be, funded by the National Institutes of Health and he has published more than 100 articles and chapters on the sleep research related topics.

In addition to his academic endeavors, he has served on the editorial boards of Sleep, the Journal of Sleep Research, the journal of Sleep Medicine Research and the journal of Behavioral Sleep Medicine. Dr. Perlis has also served as a member, or chair, of several committees and task forces of the Sleep Research Society and the American Academy of Sleep Medicine and he was a founding member of the Society of Behavioral Sleep Medicine and served as the society's first president (2010-2011).

DONN POSNER, PHD, DBSM

Donn Posner, PhD, DBSM, is the Founder and President of Sleepwell Consultants and provides consultation to organizations and individuals on a wide variety of sleep health issues including insomnia, circadian dysrhythmia, CPAP adherence, and parasomnias. He is an Adjunct Clinical Associate Professor in the Department of Psychiatry and Behavioral Sciences at the Stanford University School of Medicine and a consulting psychologist for the Palo Alto VA. Dr. Posner works on a number of grants exploring the effects of CBT-I in Gulf War Veterans, and Veterans with insomnia and Mild Traumatic Brain Injury. He is also conducting a pilot aimed at using CBT-I techniques to decrease fatigue in breast cancer survivors by increasing total sleep time. Dr. Posner spent 25 years as the Director of Behavioral Sleep Medicine for the Sleep Disorders Center of Lifespan Hospitals and was a Clinical Associate Professor in the Department of Psychiatry and Human Behavior at the Warren Alpert School of Medicine at Brown University. He is a member of the American Academy of Sleep Medicine and is one of the first Certified Behavioral Sleep Medicine specialists recognized by that group. He is a founding member of the Society of Behavioral Sleep Medicine, and he has achieved the status of Diplomate of Behavioral Sleep Medicine (DBSM).

JASON ELLIS, PHD

Jason Ellis, PhD, is a Professor of Sleep Science and Director of the Northumbria Sleep Laboratory and Clinic in the United Kingdom. He is a qualified Somnologist – Expert in Behavioural Sleep Medicine from European Sleep Research Society and a Practicing Health Psychologist under the HCPC. He splits his time between his basic research interests: the pathophysiology of sleep disorders (Insomnia, Restless Legs Syndrome, and Circadian Rhythm Disorders), the natural history of Insomnia, and his applied work on Cognitive Behavioral Therapy for Insomnia (CBT-I). Within the latter framework he examines the impact of novel adjunct therapies, the influence of social factors on adherence, and the effective delivery of CBT-I in complex cases and environments. He has worked within the National Health Service in the United Kingdom, delivering CBT-I to individuals with a range of physical and psychological conditions and serves on the editorial boards of Behavioral Sleep Medicine and Sleep Health. He has consulted for various industries and third-sector organizations and provided national guidelines to the British and Scottish governments on the identification and management of sleep problems. He is a serious advocate of public engagement and professional education with regard to behavioral sleep medicine and regularly trains CBT-I to clinicians and primary care physicians in the United Kingdom.
